Charitable Giving

Roche Diagnostics, the makers of the ACCU-CHEK® family of products, is committed to helping people with diabetes go beyond living life to loving life.

Organizations wishing to be considered for financial grant or ACCU-CHEK product donation must submit a written request. The form located below can be used to submit this request.

Funding requests will be accepted at any time and reviewed in a timely manner. Requests funded in any one year are not assured of future funding unless specified. Approved requests for product or funding will be honored a maximum of one (1) time per calendar year per individual or organization.

In addition to above, preference will be given to those programs which:

•   Have 501(c)3 tax exempt status
•   Are related to diabetes and promote the benefits of diabetes self-management, specifically compliance
•   If a national organization, their goals are relevant
•   Can be branded as an ACCU-CHEK product and promoted as such

The following funding restrictions apply to all request:

•   Requests for outside of the United States
•   Grants from organizations that are not 501(c)3
•   Individuals seeking personal help, sponsorship, or participation in events
•   Organizations that discriminate on the basis of race, color, or national origin
•   Organizations that receive funding from the United Way
•   Political organizations
•  Capital campaign
