Maximizing Coverage

Whether you have private insurance or Medicare coverage, there are ways you can make the most of your benefits. For example, just knowing what suppliers are on your insurance company's "preferred" list may help you save money each month.

For plans with different levels of prescription coverage, often different co-pays apply—the amount of money you pay out-of-pocket each time the prescription is filled. Co-pay amounts and preferred status vary from plan to plan, so it's important that you check with your insurance provider to see if the products you're using are on the preferred list. Other "non-preferred" products may also be covered, but often at a higher co-pay.

Your actual co-pay savings may be more or less than the example here.* Check your specific plan to find out the exact savings associated with a preferred brand.

Because ACCU-CHEK products are preferred by most healthcare plans, your out-of-pocket costs are often the lowest available. If ACCU-CHEK products are not a preferred brand, be sure to ask your employer or plan administrator to request that they be added.

Medicare

Even if you don't use insulin to manage your diabetes, your ACCU-CHEK testing supplies are covered by Medicare Part B and Medicare Managed Care, up to certain limits.*

Your deductible, coinsurance, insurance from other policies you may have, and co-payments, the amount you pay out-of-pocket each time a prescription is filled, will apply. Medicare will reimburse 80% of the Medicare allowed amount or 80% of the retail price of the product, whichever is less.

Using Your Coverage

1. Get your prescription.

Your doctor must prescribe blood sugar testing supplies, including how often you should test, whether you use insulin, and the quantity to be dispensed.

2. Choose your blood sugar monitoring system.

Under Medicare you may choose from any standard ACCU-CHEK system, so you can select the meter that suits your needs.

3. Find the right provider.

Medicare reimbursements must be submitted by your pharmacy or other supplier. Shop around and find a provider you're comfortable with. Make sure they are a Medicare supplier and carry the exact brand and style of meter and test strips you want. If they don't, you will want to purchase your testing supplies from another provider.

Under Medicare, you can be reimbursed for:

•  ACCU-CHEK meter systems
•  ACCU-CHEK test strips
•  ACCU-CHEK lancet devices
•  ACCU-CHEK lancets
•  ACCU-CHEK replacement batteries, when purchased separately
•  ACCU-CHEK control solution/chips, when purchased separately

Private Insurance

If you have private insurance, your insurance company may pay all or part of the cost of your diabetes testing supplies.* Review your benefit plan or contact your insurance company to find out what products and services they cover.

Diabetes testing equipment and supplies may be listed under "durable medical equipment" (DME) or prescription drugs. Be sure to check both sections for coverage. Also note whether there are any specific pre-authorization guidelines you must follow—this is especially key for supplies listed as durable medical equipment.

If you don't find diabetes equipment and supplies specifically listed among your benefits, write a formal request to your insurance company or human resources/employee benefits department to see if you can be covered for these items. A letter from your doctor explaining that these products are medically necessary may also help you obtain coverage for the supplies you need.

Patient Assistance Program

Roche Diagnostics has established a Patient Assistance Program in the United States that provides free ACCU-CHEK blood glucose test strips to people with diabetes who cannot afford them. Individuals must meet eligibility criteria in order to qualify. Patients can contact the Roche Patient Assistance Program at 866-441-4090 for information about the program and eligibility requirements.
